6. For the purposes of section 17 of the Act, the contract between the administrator of a plan and an employer or individual, as the case may be, must contain, in addition to that referred to in section 4, the following information:(1) the number assigned to it by Retraite Québec;
(2) the method for paying contributions;
(3) in the case of a contract between the administrator and an employer:(a) the frequency at which plan members may change their rate of contribution;
(b) any contribution that the employer agrees to pay into the plan;
(c) the consequences of the employer’s failure to pay employee contributions into the plan within the time frame provided for in section 59 of the Act.
